Below you will find a list of all current Pathfinder projects with a short summary of each one.

Project: Child Progress Model

Summary of Project: To support positive development of pre-school children by screening of children at 18 months, 24 months and 36 months to identify child development delay. To tailor intermediate support services as appropriate.

Project: Working with Children and Young People where Parental Substance Misuse Exists

Summary of Project: To refer parents who misuse drugs for treatment immediately and for their children to receive the appropriate support required. The objective is to increase the drug misusers awareness of the impact of their behaviour on their children and increase parenting skills.

Project: Youth Inclusion Support Panel (YISP)

Summary of Project: To aim to reduce the number of children between the ages of 8-13 years who are involved, or at risk of becoming involved, in offending and anti-social behaviour by ensuring that their families receive mainstream services/complimentary interventions from other organisations.

Project: SHAID

Summary of Project: To ensure all vulnerable 16 and 17 year olds at risk of homelessness in Derwentside receive appropriate and timely support to help prevent homelessness and where necessary to support their successful transition to independence.

Project: The Dales Emotional Well-Being Group

Summary of Project: Teesdale Schools Activity Programme - Two year project with main agencies of PCT, SC+H, CAMHS working with local schools to raise achievement through the improvement of emotional health and well-being of Teesdale children. Tow Law Community Activity Programme - Three year project - to aim to provide viable and sustainable opportunities for all young people in Tow Law so as to realise their potential.

Project: Woodhouse Close Estate Integrated Services Project

Summary of Project: The integrated project will function as a research project with the object of developing ways to co-ordinate and integrate locally delivered universal and targeted services and to provide flexible/responsive ways of accessing specialist services to meet the needs of children and their families on the estate. The project has targeted the phases 0 - 4 and 5 - 11 years inclusive and will specifically address the critical transition points of 3 and 5 years.

Summary of Project: Community work targeting issues raised and direct needs identified by the local community and practitioners who work in and around Woodhouse Close Estate and surrounding area.
